To determine your eligibility for services you must provide the following:

Current photo ID
Proof of residence (rent history or house payment receipt, utility bills, or school record)
Proof of income for all adults over 18 living in the home (pay stubs, social security statement, DHS eligibility letter, unemployment record, child support statement or income tax return)
Savings, mutual funds
Unearned Income (interest, dividends, annuity payments, stocks or benefits)
Social Security cards for everyone in your home
Medicaid card and/or insurance card
Utility bill (gas, electric, etc.)
